Our Commitment to Your Privacy

I-MAZE I INC (“Company”, “we”, “us”) is committed to protecting your privacy and the security of your Protected Health Information (“PHI”) in accordance with the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and its implementing regulations, as well as the Health Information Technology for Economic and Clinical Health Act (HITECH).

We are required by law to:

  • Maintain the privacy and security of your PHI.
  • Provide you with this Notice.
  • Abide by the terms of this Notice.

This Notice applies to all users of our website, telehealth services, consultations, and any durable medical equipment (DME) services facilitated through our platform.


How We May Use and Disclose Your PHI

We may use and disclose your PHI without additional authorization for the following purposes:

1. Treatment

To provide, coordinate, or manage your care through physicians, nurse practitioners, or other licensed professionals. This may include sending your prescriptions to a pharmacy or coordinating DME fulfillment.

2. Payment

To obtain payment for your care. We may use your PHI to determine eligibility for services, send invoices, or communicate with billing services or collection agencies. You understand all consultations are cash-pay only and we do not bill insurance for visits.

3. Healthcare Operations

For business operations such as quality assurance, audit services, regulatory compliance, training, and data analysis. This also includes communicating with vendors who help us deliver our services (e.g., cloud storage, communication platforms) under a Business Associate Agreement (BAA).


Other Permitted Uses and Disclosures

We may also use or disclose your PHI:

  • When required by law, including public health reporting, law enforcement, and national security.
  • In emergencies to prevent serious harm to you or others.
  • To coroners, funeral directors, or organ donation organizations.
  • To workers’ compensation programs.
  • For health oversight activities and audits.
  • To business associates performing administrative services on our behalf (e.g., payment processors, CRM systems).

Prescription and Pharmacy Disclosures

Physicians on our platform may refer or recommend a pharmacy based on convenience, workflow, or prior experience. However:

  • You may choose any pharmacy you prefer.
  • Some providers may have a financial interest in a recommended pharmacy. Any such relationships are disclosed as required by law.
  • I-MAZE I INC does not direct, control, or influence clinical decisions.
  • We do not receive any compensation for any pharmacy referrals.

These disclosures are intended to comply with Stark Law, Anti-Kickback Statute, and related state laws. We maintain appropriate safeguards and your freedom of pharmacy choice is always preserved.


Your Rights Under HIPAA

You have the following rights regarding your PHI:

1. Right to Access

You may request a copy of your PHI. This can be provided electronically or in paper form. We may charge a reasonable fee for copies.

2. Right to Amend

You may request corrections to your PHI. If we deny your request, we will notify you and allow you to file a statement of disagreement.

3. Right to an Accounting of Disclosures

You may request a list of certain disclosures we’ve made. This excludes routine treatment, payment, and operations.

4. Right to Request Restrictions

You may request limited use or disclosure of your PHI. We are not obligated to agree but will consider all reasonable requests.

5. Right to Confidential Communications

You may request communications via alternate methods (e.g., email instead of mail) or locations.

6. Right to a Paper Copy

You may request a paper copy of this Privacy Notice, even if you previously agreed to receive it electronically.


Electronic Communications, Digital Tools, and Consent

By using our website or platform, you consent to receive communications via email, SMS, and other digital methods. While we use secure platforms and encryption where feasible, you understand that:

  • Email or SMS may not always be fully secure.
  • You may opt out of digital communications at any time by contacting us.
  • You assume responsibility for protecting access to your own devices and login credentials.

Breach Notification

We are required by law to notify you following any breach of unsecured PHI that could compromise the privacy or security of your information. Notification may be made by email or U.S. mail.
